Please fill out the form below; starred fields are required. If you are unsure about any of the information, just give me a call or email and I’ll walk you through it. For some of the book specification information and/or jpg of the book cover, you can ask your publisher for details and materials. If you’ve purchased add-ons to the basic KidsBuzz program, be sure to fill out related areas (ex. Kidsbookclubbing, SchoolBuzz, Facebook/Instagram/Goodreads sections); and, if not, just leave those sections blank.

IMPORTANT: Don’t fill this out until you have received a booking confirmation from me with your issue dates.

NOTE: Please make sure you have all the information you are submitting saved in a text document on your computer. It is unusual but, in case there are any problems with your submission, you may have to re-enter all the information and I want you to be sure have it!
